What is an assessment?

An assessment is just a discussion about your needs, considering you as an individual and focusing on what you want to achieve. Whatever your financial circumstances, Adults, Health and Social Care can offer you a free assessment of your care needs. We will encourage and assist people to get involved with their assessment, which means discussing with you how we can best meet your needs. We will take into consideration the things that you enjoy doing, your likes and dislikes and the people you love, as well as your abilities and care needs.

To help us determine whether you are eligible for services, we use a national framework called ‘Fair Access to Care Services’. We will look at your financial circumstances and decides how much money you will be expected to contribute to any care you receive. If we are unable to offer services, we will try to help you find other ways of meeting your needs by recommending alternative organisations, helping you to maximise your own resources and by giving advice.
