Each local authority area has a Community Resource Service which brings together a range of support from Social Services, health services and third sector organisations to help people stay independent in their own homes.
Voluntary organisations in your area may also offer a time-limited hospital discharge support scheme.
People living with dementia can benefit from a range of group or individual therapies involving social interaction and mental stimulation, including reminiscence. Your local Alzheimer’s Society branch can give you information on local groups, activities and support.

If someone with dementia has to go into hospital, ask about the Butterfly Scheme which allows patients with dementia to make their personal needs and preferences clear to hospital staff.
