This single point of contact is here to give you clear information and guidance about the health and social care services available in Calderdale. When Gateway to Care is closed support is provided by the Council’s Out of Hours Team.

NHS Continuing Healthcare
NHS Continuing Healthcare is available from the NHS for people, not in hospital, who have ongoing healthcare needs. You can receive this service in any setting, including in your own home or a care home. There is no charge for NHS continuing healthcare.
NHS Continuing Healthcare is not dependent on a particular disease, diagnosis or condition, nor on who provides the care or where that care is provided.
If health care is your primary need, you may be eligible for NHS Continuing Healthcare. Once eligible for this service, your care will be funded by the NHS, (subject to review). More information is available from the Specialist Care Team at NHS Calderdale telephone 01422 281537.
