At PCCS Limited, we provide home care services and supports to adults and adults in the community. We also offer supported living service to promote services users recovery. Our specialism/ services includes; supports for people living with mental health conditions, learning disabilities, dementia, physical disabilities, personal care, sensory impairments, caring for adults under 65 yrs, and older adults over 65 yrs.
Our service users will be supported by both, in-house professionals; other local support networks, throughout the care pathway. We are uniquely placed to support people with a complex range of needs and work beyond traditional boundaries ascribed to health and social care services. Our main objective is to carryout holistic assessment of needs of potential service users employing person-centred care planning approach.
Our input is delivered through a proven approach – Person Centred Care Approach. To prevent individual service user, relapsing and to prevent unnecessary admissions to hospital or locked facilities.Person Centred Care Planning ensure that our service users are placed at the centre of the service and treated with respect and compassion as a person. The focus is on the person and what they can do, not their condition or disability. Our support focus on achieving the person’s aspiration and is tailored to the person’s and unique circumstances.We support people to build and take control over their life, supported them in developing social skills, community support, wellness initiatives, healthy lifestyles support, advocacy and support with accessing local services for people with mental health needs, learning disabilities. In order to achieve our goals, we take seriously the safety of our service users, staff and the public we are committed to enabling risk management and staff development, training.
