If you’re looking for care at home in the Sevenoaks, Gravesham and Dartford areas, we can help. We offer personalised care packages tailored to your individual healthcare needs, interests and family situation. Our care team are friendly and compassionate and are led by our Medical Director, a current NHS GP. We have a rigorous recruitment and training process so you can have peace of mind that you’ll be cared for by trustworthy caregivers. Our whole team is highly trained and experienced in supporting people with a wide range of conditions.  

 

We offer short-term care after a hospital stay, to give a family member support or to help you adjust at home after you’ve been unwell. We also offer long-term support as often or as little as you need it. When you first sign up with us, we’ll take the time to get to know you. Then we’ll tailor our care and caregivers to match what you want and need. We’ll look after all your practical, emotional and healthcare needs. It’s our mission to help you stay as independent as possible. We want you to lead an active, enjoyable and rewarding life.

 

Keeping detailed notes is a crucial part of good care delivery at home. We’ve invested in an electronic system that allows everyone in your Vitality Home Health care team to record, share and view information about your care as it happens. These notes can then be safely shared with your GP and anyone else in your healthcare team, such as a specialist nurse, occupational therapists or physios. We can also share your information with your family, if you wish. We understand how important it is to keep your loved ones updated so they feel involved in your care every step of the way.

Debbie’s career began in the ambulance service. For 11 years, she owned and operated a private ambulance service. After working for several charities overseas, Debbie returned to the UK and became involved in social care, specialising in learning disabilities. After gaining the QCF V in leadership & management qualification, Debbie was then a registered care manager for seven years. She is passionate about providing quality care with a difference, including social prescribing, lifestyle medicine and helping you live a fulfilled life.

I am so impressed with the professional approach Vitality Home Health provide. Care is tailored to the individuals needs following an in depth assessment. Nana the supervisor has kept me updated and informed throughout the whole process and is so approachable and helpful. I am particularly impressed by the Open pass app which allows me access the carers notes to ensure care is being delivered as per care plan. It can also help to identify any potential problems to escalate. All the carers have been kind and understanding with regards to my father's complex needs. Overall a really positive experience and I would highly recommend this company if you're looking for good quality care.

As a family we are very happy with vitality home health - from the start to now the service has been impeccable, and this is across the board from all staff. My uncle has been treated with care and compassion and most importantly the dignity that he deserves. This has been an incredibly difficult and emotional time for our family but vitality home health has been there to support us as-well as my uncle. I could not recommend them enough.

I just wish to say that what a fantastic help the doctor at Vitality Home Help is. My father was feeling unwell over a weekend and I made a call to the Doctor and he was so very helpful and gave great advice and also helped with a prescription that I could pickup on a Saturday afternoon. So thanks to this my father was feeling lots better on the Monday. We all know that our doctors don't work at weekends so my father would of had to wait until a week day. I'm so very grateful for his support. I would recommend the Doctor at Vitality Home Health. Thank you again.

Been with Vitality Home Health for about a month. The Care company are just amazing extremely kind and empathetic and very professional at their jobs. The carers are now like family to me especially Jas. As tragically my parents have passed away My dad died only in January 2023. I would highly recommend Vitality Home Health.
